What is a Heart Health Program?

Heart Health Program is an educational service set up to track your cardiac health and lifestyle behaviors.The Heart Health Program is designed to improve patients’ cardiovascular health and reduce the risk of heart attacks and stroke. Our program works with you to develop a personalized plan to prevent heart disease and improve your heart health. We give immediate feedback and support to help you make informed decisions about your health.

Your program may include:

  • A doctor's medical recommendations about your heart health and heart disease prevention
  • Lifestyle counseling, which may include recommendations regarding lifestyle changes, such as physical activity and nutrition
  • Patient education
  • Smoking cessation counseling
  • Stress management training

This program is open to individuals between the ages 18 of 64. It may be particularly beneficial if you have one or more of the following heart disease risk factors:

  • Family history of early heart disease or stroke
  • Diabetes or high blood sugar
  • High blood pressure
  • High or abnormal cholesterol
  • Overweight or obese
  • Physical inactivity, high stress or tobacco use
  • Prior history of an abnormal heart or vascular screening

Our team of healthcare providers perform a comprehensive cardiac evaluation during a one-day screening process. Your services include one or more of the:

  • Cardiac Consultation
  • Electrocardiogram
  • Holter Monitoring
  • Ambulatory Blood Pressure
  • Pulmonary Function Test
  • Stress Echo
  • Exercise Stress Test

How to Enrollment:

Your primary healthcare provider may refer you to the program or you may contact our office to set up an appointment without a referral.
