If you have a health concern but do not have a requisition, you can book a consultation with a healthcare provider at one of our affiliated healthcare clinics. The healthcare provider will assess your symptoms, medical history and healthcare needs to determine whether testing is appropriate. If medically required, a requisition will be provided.
Booking a consultation does not guarantee that a requisition or diagnostic test will be ordered.

Many medically necessary diagnostic tests and consultations may be covered by OHIP for eligible patients with a valid Ontario health card and an appropriate requisition. Certain services, supplies or uninsured procedures may not be covered. Our team will advise you of any applicable fees before your appointment whenever possible.
Coverage is subject to current OHIP eligibility requirements and Ministry of Health rules.
Preparation depends on the type of test you are having. You may be asked to avoid food, caffeine, smoking, certain skincare products or strenuous exercise before your appointment. You may also receive instructions regarding medications and appropriate clothing.
Our team will provide test-specific preparation instructions when confirming your appointment. Do not stop taking prescribed medication unless your healthcare provider or a qualified member of your care team specifically instructs you to do so.
